Most horror movies rely on monsters. “Iron Lung” proves the most terrifying thing is the unknown.

Released Jan. 30, the long-anticipated passion project based on the 2022 video game created by David Szymanski brings suffocating tension to the screen. Directed by and starring Markiplier, the cosmic horror film uses minimalist storytelling and psychological dread to deliver an immersive experience that lingers after the credits roll.

Set in a dystopian future after an event known as “The Quiet Rapture,” in which all known stars and habitable planets vanish, the story follows a convict named Simon. Assigned to pilot a small submarine — the Iron Lung — into an ocean of blood on a desolate moon, Simon must collect data and samples to help scientists understand humanity’s dire situation.

When he becomes stranded deep below the surface, the mission shifts from exploration to survival. With limited resources and only intermittent radio communication, Simon descends into isolation. As pressure mounts — both physically and mentally — the tension becomes nearly unbearable.

Appearing in every scene, Markiplier sustains the film’s intensity despite other characters being heard only as voices. His portrayal of fear, desperation and determination feels raw and convincing.

The film opens deliberately, establishing tone and setting before spiraling into chaos. The dialogue is sharp and purposeful. As director, Markiplier crafts an atmosphere that makes viewers feel as claustrophobic as Simon, effectively placing the audience inside the submarine.

Tight camera angles and confined framing heighten the sense of suffocation, making the vessel feel increasingly smaller as the story progresses. The heavy use of practical effects instead of CGI strengthens the realism and never feels cheap. Dark lighting and a restrained color palette reinforce the film’s unsettling tone.

The score further amplifies the tension. Eerie sound effects and carefully placed music deepen the emotional weight of each moment, creating a fully immersive experience.

“Iron Lung” explores psychological isolation, fear of the incomprehensible, class struggle and human endurance. While viewers who are unfamiliar with the original game may find parts of the narrative confusing, the film sustains a steady sense of unease.

Ultimately, “Iron Lung” delivers an intense psychological horror experience driven by performance, atmosphere and sound.
